What sort of credentialing does Encore have?

Encore is licensed by the State of Georgia and accredited by the Joint Commission as a home care provider.

Are Encore staff members available around the clock, seven days a week?

Yes. Encore services are available whenever you require care.

How can I obtain Home Care services?

Should you require skilled medical services, make an appointment to see your physician, ask your physician to fill out a Face-to-Face Referral Form and fax it to Encore at (770) 218-6966. If you want companion services, you may call Encore directly at (770) 218-6911.

How can Encore see me?

After receiving a call from you, Encore will see you on a mutually agreeable day and time. After receiving a call/referral from your doctor or case manager, Encore will call you to set up an appointment for our assessment visit.

Do I need to fill out any special paperwork?

Yes. Our staff will perform an assessment visit and will have you sign paperwork agreeing to care from Encore. Our nurse will visit you to develop a Plan of Care for the services you will receive from our staff. After each visit, you will sign documentation indicating that you received care. Encore will handle all other paperwork for you.

What sort of payment does Encore accept?

Encore accepts most Major Insurance, Private Pay, and Medicaid. Encore will help research coverage prior to the start of services.

The goal of short-term Home Care is to provide rehabilitative treatment for illness or injury. The goal of long-term home care is for the individual to maintain the highest level of safety and independence while remaining at home.
